Transplant Anesthesiology

Our Approach to Transplant Anesthesiology

Montefiore Einstein Transplant Anesthesiology is renowned for excellence in clinical care, research and education, providing state-of-the-art coordinated perioperative care for adult and pediatric kidney and liver transplantation, adult pancreas transplantation and hepatobiliary surgery. Our team of internationally-recognized transplant anesthesiologists collaborate closely with transplant surgeons, hepatologists, nephrologists, urologists, immunologists, medical and surgical oncologists, gastroenterologists, transplant pharmacists, and other transplant specialists to deliver compassionate and comprehensive patient-centered care and to ensure a safe operation and recovery. We are an integral part of the Montefiore Einstein Center for Transplantation, a global leader in the most complex transplantation cases with some of the highest survival rates in the U.S. including 100% survival of live liver donor transplant recipients. Driven by evidence-based medicine, we use the most advanced equipment and techniques to continually improve patient outcomes. As key members of the perioperative care team, we develop personalized treatment plans tailored to each patient. Our team is also instrumental in working with donors and recipients in Montefiore Einstein’s Living Transplantation Program. Montefiore Einstein is ranked in the top 1% of all hospitals in the nation for complex specialty care according to U.S. News & World Report. We also work as a unified team with our Comprehensive Cancer Center, which is among the elite 1% NCI-designated comprehensive cancer centers in the U.S.

Unmatched Expertise

Our transplant anesthesiologists have unmatched expertise for the most complex and high-risk cases. including performing anesthesia for one of the world’s first pediatric kidney transplants and performing the first double organ lung-liver transplant in New York State. We also performed the first hepatitis-positive liver transplantation into a heptatitis-negative recipient treated with antiviral therapy post-transplant. We are committed to enhancing patient safety, facilitating comfort, improving graft survival and functioning, and delivering the highest level of perioperative care to all adults and children undergoing abdominal organ transplantation and hepatobiliary surgery.

Liver, kidney and pancreas transplantation pose unique anesthetic challenges as patients requiring such surgeries often have a wide variety of complex and co-existing medical conditions. Our transplant anesthesiologists have extensive experience in the perioperative management of patients undergoing abdominal organ transplantation in adults and children using a multidisciplinary approach. All patients are carefully reviewed in a weekly meeting by a multi-disciplinary team including our anesthesiology faculty members. Participation in the comprehensive care of transplant patients enables us to deliver the best possible customized care to this high-risk patient population. Our transplant survival rates are among the best in the nation and exceed national standards.

Advanced Diagnostics & Treatments

We offer access to the latest anesthesiology techniques and technologies to help improve patient safety and provide the highest quality of care for our patients. We conduct thorough preoperative evaluations and employ the most advanced perioperative non-invasive and invasive monitoring techniques, including hemodynamic monitoring, echocardiography, and neurological monitoring, to ensure optimal outcomes for these complex surgeries.

We provide the latest evidence-based and personalized care for patients undergoing pediatric and adult solid organ transplantation and hepatobiliary surgery with particular attention to addressing potential risks associated with organ dysfunction, immunosuppression, acid-base and electrolyte imbalances, coagulopathy, and blood loss.

Our Transplant Anesthesia team provides perioperative consultation for hepatobiliary surgery and donor and recipient candidates in preparation for liver, kidney, and pancreas transplantation as well as anesthesia services for the following open, minimally-invasive and robotically-assisted surgeries.

  • Living and deceased donor liver transplantation for adult and pediatric patients
  • Hepatobiliary surgery
  • Living and deceased donor kidney transplantation for adults and pediatric patients
  • Simultaneous pancreas-kidney transplants, pancreas after kidney transplants and pancreas transplants alone for adults
  • Multi-organ transplantation for patients with simultaneous heart, liver, kidney, or lung failure such as combined liver-kidney, liver-heart, liver-lung, and kidney-lung transplantation

Clinical Trials for Transplant Anesthesiology

We are spearheading numerous clinical trials through which patients can access emerging technologies and innovative treatments that may otherwise not be available to the general public. One of the featured clinical trials we are conducting is a randomized, open-label, phase 2A study (ASCEND) evaluating the safety and efficacy of TCD601 (Siplizumab), an anti-CD2 monoclonal antibody, in combination with Belatacept and mycophenolic acid (MPA) compared to standard of care immunosuppression in de novo renal transplant recipients (ID: NCT05669001). Find a clinical trial that’s right for you.

Research

We are leading collaborative research and spearheading numerous studies and clinical trials, some of which are funded by the National Institutes of Health (NIH), to improve clinical care, outcomes and safety for patients undergoing various abdominal organ transplantation procedures. The main areas of research focus on organ preservation and machine perfusion.

The Montefiore Einstein Transplant Anesthesia team regularly contributes to peer-reviewed publications and ongoing research projects. The results of our collaborative research have been presented during national and international meetings. Our Transplant Anesthesia faculty are regularly invited to present their work at regional, national and international transplant organizations and serve as members of various committees and workgroups.
